Charge/Petition Forms

​​​Unfair Labor Practice Charge​​ - Aggrieved parties may file against public employers or unions they believe have violated their workplace rights contained under § 22-206 of the Public Employee Relations Act.​

Representation Petition​ - Public employees who wish to be represented by a union must file this form with authorization cards showing that at least 30% of employees support an election. If 50% or more authorization cards are submitted with the petition form, the union may be certified to represent those employees without an election.

Unit Clarification​ - An interested party may file this form to challenge a public employer's determination of which employees are included in a bargaining unit.

Impasse Request - Either or both parties in bargaining may file this form if they believe they have reached a deadlocik over a mandatory subject of bargaining, such as waes, hours, or working conditions,.

Negotiability Dispute (Public School Employees/Employers only) - Public school employers and unions may file this form to receive a determination from PERB on whether a subject of bargaining is mandatory, permissive, or illegal.

Decertification of Representative​ - File this form with PERB if an employee/group of employees does not want to be represented by the union representing them​. Must include at least 30% showing of interest.
